Eligibility Requirements for Employees
- Must be a W-2 employee of a business located within Galveston County
- Qualified employer must enroll in UTMB Multi-Share Plan before employee may enroll
- Employee must work a minimum average of 20 hours per work week
- May not be covered by any group insurance plan
- Employee may not currently eligible for, or covered, under any government-sponsored health benefits plan, such as Medicare, Medicaid, or Veterans Medical benefits
- Employees earning less than $18.40 per hour may be eligible for premium assistance (if funding available)
Eligibility Requirements for Employers
- Located within Galveston County
- In business for at least one year prior to enrollment
- Have between 2 and 50, W-2 employees
- 50% of eligibility employees are required to participate
