A patient who dies after having been admitted to John Sealy
Hospital, League City Campus and Angleton Danbury is considered an
inpatient death. UTMB offers a valuable service to all our families at
the time of death of an autopsy at no charge for all in-patient deaths.
Following the death of a patient at UTMB hospitals, the
pronouncing physician, Nursing Service and Autopsy Service completes
all sections of the FORM 5012.
The pronouncing physician is responsible for contacting the
authorizing next-of-kin to discuss consent to postmortem examination
(autopsy), disposition of the body and disposition of organs, fluids,
prosthetic devices and tissue after postmortem examination (autopsy)
with the authorizing next-of-kin, sections: 1, 2, 3A, 3B, 3C, 4 and 5A.
Nursing Service will assist the pronouncing physician and review
the FORM 5012 before the decedent is transferred to the morgue or
released from the hospital unit, completing out section 5B.
