Our patient care team looks forward to providing outstanding health care to you and your family. As part of your health benefit through UTMB Health, you and your dependents will receive:

  • Preferred access to our Minor Injury and Illness Clinics with a $15 fee per visit
  • Preferred access to the Chronic Care Assessment and Coordination Clinic with a $15 fee per visit for high blood pressure, diabetes, etc.
  • Same-day appointments (subject to medical plan primary care copayment) at all urgent care locations and, if not same-day, within 24 hours at many primary care locations throughout the service area
  • Annual Health Risk Assessment (AHA) labs and an in-person or tele-consult with a nurse to review your results
  • UTMB Access Services support that is available 24/7, 365 days a year to schedule appointments and provide nurse triage services

Chronic Care Coordination

Adults who have chronic health conditions, such as hypertension (high blood pressure) or diabetes, may schedule an appointment to receive an assessment from a UTMB provider. This provider will manage their care (when appropriate) or make a referral to a UTMB specialist as needed to properly manage the chronic condition.

Appointments for the Chronic Care Assessment and Coordination Clinic are available within three business days by appointment at the Galveston location only.

Annual Health Assessment (AHA) Lab Draw

Current employees and retirees enrolled in any of the County’s medical plans have between January 1 - October 31 every year to complete an Annual Health Assessment in order to secure the lower premium rates.

The AHA consists of a scheduled visit for a panel of labs to be drawn, followed by an in-person or telehealth visit with our clinical team to review your results and provide guidance if further evaluation is needed.

Post AHA Lab Results Review

After you receive the results of your Annual Health Assessment lab, you can schedule a personalized visit with a health care provider. This one-time visit is part of your health benefits through UTMB Health. During this one-on-one session, your provider will:

  • Discuss your lab results in detail.
  • Identify potential health risks and interventions.
  • Provide guidance on improving overall health and well-being.
  • Help you take proactive steps toward maintaining and enhancing your health.
