Chronic Diseases Prevention & Self-Management ProgramsFREE Evidence-Based Programs

Hanaa S. Sallam, MD, PhD, and Hani Serag, MD, MPH are Research Fellows with the Sealy Center on Aging. The husband-and-wife team are the Principal Investigators for UTMB’s Chronic Diseases Prevention & Self-Management Programs. These educational programs serve diverse populations and are tailored to our older adult community. They are supported by the Texas Department of State Health Services and the Administration for Community Living Health and Human Services.

  1. Diabetes Prevention Program (DPP): The evidence-based National Diabetes Prevention Program using the CDC’s accredited PreventT2® curriculum features an approach that is proven to stop/delay prediabetes from progressing into type 2 diabetes. If you have prediabetes, these lifestyle changes can cut your risk of developing type 2 diabetes in half. By improving food choices and increasing physical activity, you can lose 5 to 7 percent of your body weight. This program is made available for adults with prediabetes. Meet with your Certified Lifestyle Coach for 1 hour weekly-bi-weekly in the first 6 months, then once monthly for the remainder of the year in person or on Zoom.
  2. Intensive Diabetes Self-Management Education and Support (iDSMES): Put together by Dr. Hanaa Sallam at UTMB, this program uses evidence-based modules to augment diabetes self-management among people with type 2 diabetes through improving related knowledge, skills, and self-efficacy. This program is made available for adults with type 2 diabetes. Meet with your Certified Lifestyle Coach for 1 hour weekly-bi-weekly in the first 6 sessions, then once monthly for the remainder of the year in person or on Zoom.
  3. Chronic Disease Self-Management Program (CDSMP): an evidence-based program developed at Stanford University Patient Education Research Center as a collaborative with the Northern California Kaiser Permanente Medical Care Program, this program is proven to help participants with chronic co-existing conditions learn decision making, better breathing, dealing with pain, dealing with difficult emotions, and more. This program is made available for people 65 years of age and older. Meet with your Certified CDSMP Leader for 2.5 hours once weekly for 6 weeks in person, or live online.
  4. Diabetes Self-Management Program (DSMP): Developed at Stanford University Patient Education Research Center as a collaborative research project with the Northern California Kaiser Permanente Medical Care Program, this program is proven to help people with diabetes learn the necessary skills to manage their diabetes through healthy eating, stress management, and more. This program is made available for people 65 years of age and older with diabetes. Meet with your Certified DSMP Leader for 2.5 hours once weekly for 6 weeks in person, or live online.
  5. Toolkit for Active Living with Chronic Conditions: This program is proven to help participants with chronic co-existing conditions learn decision-making, better breathing, dealing with pain, dealing with difficult emotions, and more. This program is made available for people 65 years of age and older with any chronic disease, including but not limited to, diabetes and hypertension, heart disease, stroke, or arthritis. Receive a toolkit by mail and chat with your Certified Leader and peers for 1 hour once weekly for 6 weeks in person, over the phone, or live online.
  6. Health Coaches for Hypertension Control (HCHC): Developed by the Clemson Institute for Engaged Aging, this program has been proven to help people with high blood pressure better manage their condition by focusing on nutrition, physical activity, stress management, and more. Trained facilitators guide the small-group program and participants receive a free electronic blood pressure monitor and other educational materials. This program is made available for people 65 years of age and older with hypertension. Meet with your Certified HCHC Leader for 1.5 hours weekly for 8 weeks in person, or live online.
  7. Geri-Fit®: A strength bodybuilding program. Most exercises are seated. All exercises are safe for all levels of fitness. This program is made available for people 65 years of age and older. Meet with your Certified Geri-Fit® Instructor for 45 minutes twice weekly for 12 weeks in person, or live online.
  8. Walk With Ease (WWE): Developed by the Arthritis Foundation, this program has been proven to help people with arthritis engage in walking, stretching, and strengthening exercises to reduce pain, increase balance, and strength, and build confidence in their ability to be physically active. This program is made available for people 65 years of age and older. Meet with your Certified WWE Leader for 45 min thrice weekly for 6 weeks in person or receive the book and work at your own pace checking in with your leader periodically.
