OTCAS Supplemental Application

Thank you for applying to the University of Texas Medical Branch (UTMB Health), School of Health Professions, Doctor of Occupational Therapy Program through the Occupational Therapy Centralized Application Service (OTCAS). Your Application ID is required to access the Supplemental Application. It can be found in the email correspondence you received containing the application link. If you need assistance obtaining your Application ID or assistance with this form, please contact The Office of Enrollment Services at 409-772-1215.

To access your Supplemental Application, enter your Application ID below and select the Submit button. Once submitted, your Application ID will be verified.
