| Specimen Requirements: | Special Instructions: 1. Submit FRESH only, absolutely no fixative or saline. 2. Patient's chart is required. 3. Contact Surgical Pathology when frozen section is pending and/or specimen requires opening. 4. Surgical Pathology fellow/resident is available for intra-operative consultation. |
| Test Included: | Tissue removed in surgery for STAT diagnosis. Gross and microscopic examination with diagnosis. |

| Container: | Clean container of sufficient size to support the tissue. |
| Storage/Transport: | Specimen must be ordered through EPIC/Beaker as a frozen section specimen. The specimen is collected in a clean container labeled with the patient's first and last names, unit history number and specimen identification. |
| Specimen Preparation: | Any tissue, except bone. Lymph node for metastatic disease or to identify tissue, but not for lymphoma diagnosis. |
| Causes for Rejection: | Order not submitted in Beaker, insufficient identification, incomplete information, fixative added to sample, contaminated container. |
