Patient information form must be completed. 4. Orders must be placed in Beaker and must include the physician's name, ID number, signature, clinical history and diagnosis. |

Container: | Vial with 2% glutaraldehyde plus 2% paraformaldehyde or Zamboni obtained from Surgical Pathology Gross Room, 4.414, Clinical Services Wing. |
Storage/Transport: | Tissue is placed in 2% glutaraldehyde plus 2% paraformaldehyde or Zamboni soluction which is available in the Surgical Pathology Gross Room, 4.414 Clinical Services Wing, Specimen should be labeled with patient's name, unit history number and specimen identification. The specimen should be delivered immediately to Surgical Pathology Gross Room, 4.414, Clinical Services Wing. Short term storage 4º C if necessary. DO NOT FREEZE. |
