Medical Waste Pickup
Rejection Checklist

The bright yellow rejection checklists will be utilized by Environmental Services (EVS) staff across all UTMB campuses to assist with the proper packing and handling of medical waste.

The checklists will also explain the reason(s) why a particular box was not picked up.

For questions or clarifications, please contact Environmental Health and Safety (EHS) at (409) 747-0515 and ask for Environmental Protection Management (EPM).

Below are hazardous waste label links with all of the required information for you to use to make labels. 

You can also open MS Word (under the tab "Mailings", then "Labels"), copy the image below and paste into the blank page of labels (e.g., Avery Labels), center the columns, and save it as your label template.

Hazardous Waste Pickups Information

  • Controlled Substances
    Controlled substances are handled differently than regular chemicals.  Please allow EPM to reach out and schedule with you.  Remember: the license holder must be present. 

  • Galveston Campus
    Hazardous waste pickups will be completed within 72 hours of receipt of chemical pickup request following requirements for Large Quantity Generators of hazardous waste.
  • League City Campus
    Hazardous waste pickups will be completed within 72 hours of receipt of chemical pickup request following requirements for Small Quantity Generators of hazardous waste.
  • Clear Lake Campus 
    Hazardous waste pickups will be completed within 72 hours of receipt of chemical pickup request following requirements for Small Quantity Generators of hazardous waste.
  • Angleton Danbury Campus
    Hazardous waste pickups will be completed after correspondence between EHS and requestor following requirements of Small Quantity Generators of hazardous waste.
  • Offsite Clinics
    Hazardous waste pickups will be completed after correspondence between EHS and requestor following requirements of Very Small Quantity Generators of hazardous waste.
