“Trends in Revascularization and Utilization of Mechanical Circulatory Support in Elderly Patient Presenting with Acute Myocardial Infarction Complicated by Cardiogenic Shock”
Acute myocardial infarction (AMI) complicated by cardiogenic shock carries a high morbidity and mortality rate. Despite this, there is paucity in real world data on outcomes in elderly patients (>65 years) presenting with ST-elevation myocardial infarction (STEMI) complicated by cardiogenic shock. Therefore, we performed a retrospective cohort study using the 100% Texas MEDPAR database from 2005-2021 to examine temporal trends in utilization of invasive therapies such as fibrinolytics, PCI, CABG, mechanical circulatory support, and pulmonary artery catheterization in patients 65-75 years and >75 years. We also evaluated in-hospital mortality, length of stay, and 30-day readmission. We found an overall increase in the utilization of PCI, Impella, and pulmonary artery catheterization with a decline in IABP and CABG use. There was decreased in-hospital and 30-day mortality in elderly patients who underwent revascularization.
Dr. Thakker is a 2nd year fellow in the Division of Cardiovascular Medicine. He is an alumnus of the UTMB School of Medicine and Internal Medicine Residency Program where he was awarded the GME overall outstanding resident award. His clinical interests are in cardiogenic shock and invasive therapies in the treatment and management of AMI cardiogenic shock. His research interest(s) are focused on outcomes research and quality improvement. He has published 38 publications during his training in peer-reviewed journals such as JACC Cardiovascular Interventions, JAMA Network Open, Journal of the American Heart Association, and others. Aside from his clinical and research work, he is active in leadership at the national level for the American College of Cardiology.
